­­­Here’s how you can get your referral vouchers:

­Step 1: Please, remind them to mention YOUR full name when completing the online “New Patient Forms.” This is a required step for all new patients, who must fill out these forms either online or in person before their scheduled appointment.
  • We’ve included a “HOW DID YOU HEAR ABOUT US?” question in our new patient forms.
  • When completing this section, if you select “Someone I know is a patient here,” a follow-up question will appear: “MAY WE KNOW WHO REFERRED YOU?“
  • In the space provided, please enter the first and last name of the existing patient who referred you to our practice.

Step 2: After their appointment is complete, we’ll put a $50 OFF and $100 OFF vouchers on both of your accounts towards future dental treatment.

HOW IT WORKS
  • An Existing Patient refers a friend or family member to our office, and when the New Patient referral completes their New Patient Exam (Comprehensive Exam, Full Mouth Series X-Rays, Intra-Oral Images) and MINIMUM ONE RECOMMENDED TREATMENT, BOTH will receive a “$50 OFF” and “$100 OFF” vouchers in each of their accounts to be used for future treatment!
  • We keep it simple !! The $50 OFF and $100 OFF Vouchers apply towards “out of pocket” costs of dental treatment. 
  • When patient spends $350 out-of-pocket on dental treatment, a discount of $50 may be applied to that amount.
  • For an out-of-pocket expenditure of $700 or more, a discount of $100 is applicable.­
­Terms and Conditions

  1. A “Referral” applies to New Patients only – existing patients or patients who have previously visited our office do not apply.
  2. “Share a Smile, Refer a Friend” form must be completed and presented at initial visit (prior to New Patient Exam) and cannot retroactively be applied.
  3. No cash value and non-transferrable – the $50 OFF and $100 OFF vouchers may only be used towards future treatment and cannot be used towards copayments, deductibles or products. Cannot be used for previous balances or combined with any other offers or towards purchasing our Wellness Plan.
  4. New Patient “Referrals” must complete their New Patient Exam (Comprehensive Exam, Full Mouth Series X-Rays, Intra-Oral Images) as well as AT LEAST ONE RECOMMENDED TREATMENT for vouchers to be received by both parties.
  5. Referring patient must be an active patient who has been seen at Divine Dental in the last two years.
  6. Must be 18 years or older to receive $50 OFF & $100 OFF vouchers. Parents or Legal guardians can apply it towards ‘out of pocket’ costs for their kids.
